Please complete this form to give Chemuniqué permission to use photographs and/or video recordings of yourself, your business, and/or staff in various Chemuniqué publications, including, but not limited to websites, social media, newsletters, pamphlets, and marketing materials, without any compensation to you.

We may share the material with other organisations for marketing and communication projects, such as providing images to design or print agencies.

News media and their associated websites, social media channels, print publications, television, and radio.

By giving consent, Chemuniqué has the right to copyright the photographs and/or video recordings, in whole or in part, and to use, re-use, publish or re-publish them as described. This permission applies regardless of any changes or alterations made to the original content.

I acknowledge that I have no right to receive any royalties or compensation arising from or related to the use of photographs and/or video recordings by Chemuniqué.

Unless specified by me, Chemuniqué has my permission to take photographs or video recordings of anything at the location referred to above.
